You may reach the Appointment Desk by dialing 850.595.6554 or visit the Escambia County Health Department main facility
(1295 West Fairfield Drive, Pensacola) Monday through Friday from 8:00 am
until 5:00 pm. The Appointment Desks are located in the yellow section
of the main hallway.
Please call 850.595.6554 for additional
information on Eligibility. After you have scheduled your Eligibility
appointment, the clerk will inform you of any documents you need to bring
for your appointment.
The $10 fee entitles the applicant to one
computer certification of a registered birth.

If photocopy is requested in lieu of a
computer certification, an additional charge of $5.00 is required, making
the first photocopy fee $15.00
Additional copies of the
same type certification ordered above are $6.00 each when ordered with the
request.

To receive dental services you must first
go through the central eligibility department. You can make an appointment
for an eligibility appointment by going to the yellow appointment desk in
the central hallway or you can call 595-6554 to make an
appointment. If you or your child is in pain, an emergency appointment
consisting of an x-ray and possibly a prescription for infection can be
obtained.
Children and Adults
We
provide comprehensive dental treatment to children ages 3 to 20 who are
Medicaid eligible.
We provide limited services to adults on Medicaid.
Emergency treatment and extractions are the only services offered.
Children who are not on Medicaid can receive an emergency appointment
only. This includes an x-ray and prescription if necessary, until the
child can get to a private dentist. No actual dental work can be
performed on child. If patient has gone through eligibility the fee is
based on income. If patient has not gone through eligibility, we can
provide this service for a $20.00 fee.
If
you or your child is in pain, and are Medicaid eligible, an appointment
consisting of an x-ray and possibly a prescription for infection can be
obtained prior to eligibility.
